Getting started

SAME DAY ACCESS 

Are you seeking mental health or substance use services? Do you need linkage with intellectual or developmental disability services?

Our Same Day Access (SDA) program is the initial point of contact for information and referrals for services offered at District 19 Community Services Board. No Appointments are required. You can call for more information or walk in to our locations to see a clinician during the days, hours, and locations identified below.

We are here to serve you. Sometimes, help takes time. The assessment process may take a minimum of two hours. To prepare for your appointment, please have this information available, if applicable: 

  • A picture ID
  • Social Security Card
  • All insurance cards, including Medicaid and Medicare. If you are uninsured, please be prepared to provide proof of income at time of service (e.g. pay stubs, social security benefits statements, unemployment, self-employment/1099, W2 forms, retirement, pension)
  • Co-payment or co-insurance, if applicable
  • Legal guardian or custodial documentation
  • Note: If bringing children, you will be asked to stay with your child
  • List of current medications
  • Name, address and phone number of emergency contact
  • Name, address and phone number of current and past treatment providers
  • If seeking, ID/DD services, an updated psychological evaluation should be brought or forwarded to District 19, CSB
  • If seeking following up services after hospitalization, hospital discharge paperwork should be brought or forwarded to Distrcit 19 CSB.
