Individuals with intellectual/developmental disabilities are often challenged by a complicated service delivery system as they seek assistance in meeting their physical, social and emotional needs. Case managers assist individuals in determining eligibility for services and then work collaboratively with individuals and community partners to plan services which enhance the individual's quality of life. Throughout the stages of each person's life, the case manager monitors the delivery of services and advocates for consumers when needed.
Intellectual disability services are available for individuals age 22 and above. For younger individual, please contact our Child & Adolescent Services division.
