Program participants will receive free breakfast, lunch and snack based on the following requirements:

Must have an open case with District 19 Community Services Board enrolled in one of the Mental Health Day Programs i.e. Atlantic House

Community Day, Spring Center or Sycamore Center. Program participant must have a disability i.e. co-occurring or mental health diagnosis which substantially limits one

or more major life activities. Eligibility for participation in the USDA program is based on the enrolled participant receiving, food stamps, social security disability, or

medicaid. Enrolled participant will be categorized by free, reduced or paid, however

MONETARY FUNDS ARE NOT ACCEPTED!!!


Program Requirements:

Reasonable accomodations will be made for program participants with diet restrictions due to medical conditions. A physicians note must be submitted

to support the change in meal accomodations. Language assistance will be available as needed for translation.
