What is Intensive Care Coordination, and is my child eligible for this service?

The overall purpose of Intensive Care Coordination Services is to provide the coordination of comprehensive community resources and services to children and adolescents who are at risk of entering or who are currently placed in a residential facility.  Intensive Care Coordination (ICC) services are short-term services provided to those children and adolescents who would benefit from a higher level of intensity than can be provided by typical case management services.  Services and resources may be formal or informal, and can include educational, financial, legal, vocational, medical, psychiatric, psychological, and social support opportunities.

Children who are eligible for Intensive Care Coordination Services are identified and assessed by the local Family Assessment and Planning Teams (FAPT); those who are eligible for ICC services are referred to D19 Intensive Care Coordinator for further assessment/services.

How do I refer a student to the School Based Therapy Program?

A referral can be made by an administrator, parent/guardian, school counselor, student (self), or teacher through the guidance department.  Once the school counselor receives the referral, he/she will contact the parent(s)/guardian(s) or legal representative to gain consent for the student to participate in the School Based Therapy Program.  Once the School Counselor receives the appropriate consent, he/she will contact the School Based Therapist with the referral.  The School Based Therapist will then set up a time to begin the assessment process with the student.
